Our commitment to your privacy
How we may use and disclose your health information
Treatment. We may use your health information to provide, coordinate, or manage your care, including consultation with and supervision by Rhonda Septelka, LPC-S (TX #76324).
Payment. We may use and disclose your health information to obtain payment for services, such as providing a superbill at your request for out-of-network reimbursement.
Health care operations. We may use your health information for quality assessment, supervision, training, and business operations of the practice.
Uses and disclosures that require your written authorization
Uses and disclosures that may be made without your authorization
The law permits or requires us to use or disclose your health information without your authorization in limited circumstances, including:
- When required by federal, state, or local law.
- To prevent a serious and imminent threat to your health or safety or that of another person.
- For certain public health activities.
- To report suspected abuse, neglect, or domestic violence, as required by Texas law.
- In response to a court order, subpoena, or other lawful process.
- For health oversight activities, including licensing board oversight.
Your rights regarding your health information
- Right to request restrictions on certain uses and disclosures.
- Right to request confidential communications by alternative means or at alternative locations.
- Right to inspect and copy your health information, subject to certain limits.
- Right to request an amendment to your health information.
- Right to an accounting of certain disclosures.
- Right to a paper copy of this Notice upon request.
Breach notification
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us at contact@junebridgecounseling.com, or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be penalized or retaliated against for filing a complaint.
